Delivering care for the resident with physical challenges in a supportive environment is an essential part of the Country Meadows community. Through Pathways Restorative Services (formerly the William Penn Program), we offer therapy and rehabilitative programs that enable residents to lead active and independent lives. We help residents establish goals for greater physical endurance and independence. We meet them halfway by providing structure, programs, support and encouragement. We accomplish this by identifying factors that will motivate residents to participate successfully in our program.
Pathways Restorative Services:
- Meets the needs of residents who require either rehabilitative care or permanent residency, when chronic conditions exist.
- Is ideal for the individual who requires more rehabilitative care/support services than are provided in Assisted Living, but who does not need nursing home care.
- Evaluates residents to develop personal care plans that address individual needs through specially designed programming.
- Offers an active community life program that includes social events, spiritual life services, recreational activities and cognitive and intellectual stimulation.
- Circles of Life, a spiritual life support group, offers residents a place to share their life story and engage in healthy self reflection.
Is successful in helping residents learn that they still have much to contribute.
